Statement of Loss
This claim, be valid according to applicable tariff provisions, must be filled with Fiji Airways 21 days of date of loss. Claim is hereby made against Fiji Airways in the
amount of $FJD, as set forth herein, for a loss occurring on   I do hereby warrant the foregoing statement and any statements on this form to be accurate, complete and true.
Normal claim resolution is 28-35 days after receipt of the requested items listed above.
PLEASE DO NOT FORGET TO INCLUDE THE REQUESTED DOCUMENTS INCLUDING ALL PURCHASE RECEIPTS/PROOF OF OWNERSHIP AND COPIES OF YOUR TICKET AND BAGGAGE TAG.
